Media Consent and Release of Liability

Throughout the academic year, students may be featured to promote HCCS activities and achievements. This may include appearances on television, videos, websites, DVDs, brochures, and other forms of media.

By signing this form, I hereby grant the Office of Student Life/HCCS permission to use my photographs, interviews, and likenesses in audio, film, video, and other electronic, digital, and printed media. I release the Office of Student Life/HCCS and its employees from any liabilities, known or unknown, arising from the use of this material.
